Automatic registration of 3D models to laparoscopic video images for guidance during liver surgery
Laparoscopic liver interventions offer significant advantages over open surgery, such as less pain and trauma, and shorter recovery time for the patient. However, they also bring challenges for the surgeons such as the lack of tactile feedback, limited field of view and occluded anatomy. Augmented reality (AR) can potentially help during laparoscopic liver interventions by displaying sub-surface structures (such as tumours or vasculature). The initial registration between the 3D model extracted from the CT scan and the laparoscopic video feed is essential for an AR system which should be efficient, robust, intuitive to use and with minimal disruption to the surgical procedure. Several challenges of registration methods in laparoscopic interventions include the deformation of the liver due to gas insufflation in the abdomen, partial visibility of the organ and lack of prominent geometrical or texture-wise landmarks. These challenges are discussed in detail and an overview of the state of the art is provided. This research project aims to provide the tools to move towards a completely automatic registration. Firstly, the importance of pre-operative planning is discussed along with the characteristics of the liver that can be used in order to constrain a registration method. Secondly, maximising the amount of information obtained before the surgery, a semi-automatic surface based method is proposed to recover the initial rigid registration irrespective of the position of the shapes. Finally, a fully automatic 3D-2D rigid global registration is proposed which estimates a global alignment of the pre-operative 3D model using a single intra-operative image. Moving towards incorporating the different liver contours can help constrain the registration, especially for partial surfaces. Having a robust, efficient AR system which requires no manual interaction from the surgeon will aid in the translation of such approaches to the clinics

sportivă și a primei asociaţii de profil.
Primul Congres de medicină sportivă a avut
loc la Amsterdam, Niderlanda, în august 1928. La
el au participat specialiști în domeniul medicinei
sportive din peste 20 de ţări, abordând tematica
stării funcţionale a organismului la sportivi în baza
datelor antropometrice, stării somatice a sistemului
cardiovascular și datelor stării metabolismului după
parametrii biochimici. În anul 1933, la Torino, Italia,
la Congresul II Internaţional de medicină sportivă a
fost înfiinţată Federaţia Internaţională de Medicină
Sportivă (FIMS)
Some considerations regarding the treatment of digestive non-Hodgkin lymphomas

Orice organ sau țesut este predispus la dezvoltarea Limfoamelor non-Hodgkin (LNH), tractul digestiv fiind cea mai frecventă localizare pentru limfomul extraganglionar. Din cauza simptomelor nespecifice, diagnosticul și aplicarea unui tratament ce ar permite un control bun al tumorii, poate fi întârziat. Scopul lucrării. Prezentarea datelor din privind unele considerații în tratamentul LNH digestive. Material și metode. Au fost studiate aspectele clinice, diagnostice și de tratament la 37 de pacienți cu LNH digestive din cadrul Institutului Oncologic din Moldova. Toți pacienții au fost supuși examenului clinic și o evaluare obiectivă prin investigații paraclinice de laborator, imagistice și endoscopice. Tratamentul de care au beneficiat pacienții a fost chimioterapic, chirurgical, sau combinat și complex. Rezultate. S-a constatat că, majoritatea (38%) pacienților cu LNH digestive aveau vârste cuprinse între 51 și 60 de ani. Localizarea întâlnită cel mai frecvent a fost zona gastrică și chiar dacă leziunea esofagiană este identificată excepțional de rar, în această perioadă a fost întâlnit un caz. Conform studiilor, cei mai mulți pacienți au fost diagnosticați în stadiul IV, demonstrând o adresabilitate tardivă și un prognostic nefavorabil în perspectivă. Chimioterapia a fost cel mai utilizat tip de tratament, urmat de combinarea sa cu intervenția chirurgicală. Cinci cazuri (14%) au fost supuse unui tratament complex, radioterapia fiind folosită ca mijloc de consolidare a efectului, cu un control bun al tumorii sau cu scop paliativ. Concluzii. Intervențiile terapeutice pot varia, în funcție de numeroși factori: localizare, stadiu, dimensiunea tumorii, subtipul de limfom, anumite simptome, vârsta și statusul de performanță. Tratamentele combinate și complexe îmbunătățesc ratele de supraviețuire, cresc eficacitatea terapiei și reduc frecvența recăderilor.Background. Any organ or tissue is predisposed to the development of Non-Hodgkin’s Lymphoma (NHL), the digestive tract being the most common location for extranodal lymphoma. Because of the non-specific symptoms, the diagnosis and the application of a treatment that would allow a good control of the tumor, may be delayed. Objective of the study. Presentation of data from regarding some considerations in the treatment of digestive NHL. Material and methods. The clinical, diagnostic and treatment aspects were studied in 37 patients with digestive NHL from the Moldovan Oncological Institute. All patients underwent a clinical examination and an objective assessment through paraclinical laboratory, imaging and endoscopic investigations. The treatment that the patients benefited from was chemotherapeutic, surgical, or combined and complex. Results. It was found that the majority (38%) of patients with digestive NHL were aged between 51 and 60 years. The most frequently encountered location was the gastric area, and even if the esophageal lesion is identified exceptionally rarely, one case was encountered during this period. According to the stages, most patients were diagnosed in stage IV, demonstrating a late addressability and an unfavorable prognosis in the future. Chemotherapy was the most used type of treatment, followed by its combination with surgery. Five cases (14%) were subjected to a complex treatment, radiotherapy being used as a means of consolidating the effect, with good tumor control or for palliative purposes. Conclusion. Therapeutic interventions may vary, depending on numerous factors: location, stage, tumor size, lymphoma subtype, certain symptoms, and age and performance status. Combined and complex treatments improve survival rates, increase the effectiveness of therapy, and reduce the frequency of relapses
Data-centric multi-task surgical phase estimation with sparse scene segmentation
PURPOSE: Surgical workflow estimation techniques aim to divide a surgical video into temporal segments based on predefined surgical actions or objectives, which can be of different granularity such as steps or phases. Potential applications range from real-time intra-operative feedback to automatic post-operative reports and analysis. A common approach in the literature for performing automatic surgical phase estimation is to decouple the problem into two stages: feature extraction from a single frame and temporal feature fusion. This approach is performed in two stages due to computational restrictions when processing large spatio-temporal sequences. METHODS: The majority of existing works focus on pushing the performance solely through temporal model development. Differently, we follow a data-centric approach and propose a training pipeline that enables models to maximise the usage of existing datasets, which are generally used in isolation. Specifically, we use dense phase annotations available in Cholec80, and sparse scene (i.e., instrument and anatomy) segmentation annotation available in CholecSeg8k in less than 5% of the overlapping frames. We propose a simple multi-task encoder that effectively fuses both streams, when available, based on their importance and jointly optimise them for performing accurate phase prediction. RESULTS AND CONCLUSION: We show that with a small fraction of scene segmentation annotations, a relatively simple model can obtain comparable results than previous state-of-the-art and more complex architectures when evaluated in similar settings. We hope that this data-centric approach can encourage new research directions where data, and how to use it, plays an important role along with model development

CLINICO-MORPHOLOGICAL ASPECTS OF AGGRESSIVE NON-HODGKIN’S LYMPHOMAS

Limfoamele non-Hodgkin (LNH) agresive reprezintă a 6-a cauză de deces în structura tumorilor maligne în țările europene și SUA. Depistarea patologiei în stadiile ei inițiale și identificarea tipului morfologic sunt elemente esențiale în alegerea tacticii de tratament. Scopul lucrării. Studierea aspectelor clinico-morfologice a LNH agresive pentru a identifica unele particularități de evoluție clinică. Material și Metode. Au fost studiate aspectele clinico-morfologice la 54 de pacienți cu diagnosticul de limfom non-Hodgkin agresiv cu vârsta de la 18 până la 85 de ani(bărbați-24, femei-30). Diagnosticul a fost confirmat morfologic și imunohistochimic. Rezultate. LNH agresive s-au dezvoltat mai frecvent la persoanele cu vârsta de 40-64 de ani(57,4%). Mai des a fost diagnosticat tipul morfologic limfoblastic (64,8%), preponderent la vârsta de peste 65 de ani(80,0%). LNH difuz din celulă mare B a fost constatat în 29,6% cazuri, mai des la vârsta de 18-39 ani(62,5%). Rar au fost diagnosticate LNH zonei de manta(3,8%) și LNH anaplazic (1,8%). Debutul LNH agresive a avut loc în nodulii limfatici periferici în 63,0% cazuri, extranodal-37%. La etapele inițiale, în majoritatea cazurilor (68,8%), LNH s-a răspândit în nodulii regionali. Metastaze extranodale mai des au fost determinate în splină(51,7%), măduva oaselor(34,5%), ficat(31,0%). Concluzii. Particularitățile clinico-morfologice evidențiate ale LNH agresive vor contribui la optimizarea conduitei de diagnostic și tratament a pacienților cu LNH agresive.Background. Aggressive non-Hodgkin’s lymphomas (NHL) are the 6th leading cause of death among malignant tumors in European countries and the USA. Detection of the disease in its early stages and identification of the morphological type are essential elements in the therapeutic approach. Objective of the study. To study the clinico-morphological aspects of aggressive NHL in order to identify some peculiarities of the clinical course. Material and Methods. Clinico-morphological aspects were studied in 54 patients diagnosed with aggressive non-Hodgkin’s lymphoma aged 18 to 85 years (male-24, female-30). The diagnosis was confirmed morphologically and immunohistochemically. Results. Aggressive NHL occurred more frequently in patients aged 40-64 years (57.4%). The most often diagnosed morphological type was lymphoblastic NHL (64.8%), predominantly in people over 65 years old (80.0%). Diffuse large B-cell NHL was found in 29.6% of cases, mostly at the age of 1839 years (62.5%). Seldom were diagnosed variants such as mantle cell NHL (3.8%) and anaplastic NHL (1.8%). The onset of aggressive NHL occurred in peripheral lymph nodes in 63.0% of cases, extranodal in 37%. In the early stages, in the majority of cases (68.8%), NHL spread to regional nodules. Extranodal metastases frequently were determined in spleen (51.7%), bone marrow (34.5%), and liver (31.0%). Conclusion. The highlighted clinical morphological features of aggressive NHL will contribute to optimizing the diagnostic and treatment management of patients with aggressive NHL
Клинические проявления и частота рецидивов у больных с локальными стадиями лимфомы Ходжкина
Clinical manifestations and frequency of relapses were studied in 90 patients, aged 18-79 years, with Hodgkin’s lymphoma (HL), classical type, stages I and II, with complete remission after the first-line treatment. The diagnosis in all cases was morphologically confirmed. It was found that HL developed more frequently in people aged 18-39 years (76.7%). In 55.5% cases, the diagnosis was established within the first 3 months of the first manifestation of the disease.The onset of HL, classical type, occurred more frequently in the mediastinum (42.3%) and supraclavicular lymph nodes (33.3%). The frequency of recurrence accounted for 24.4%. Relapses occurred more frequently in cases of primary involvement of the mediastinum and supraclavicular lymph nodes (26.3% and 26.7%, respectively), depending on the tumor size. Relapses were recorded more frequently in HL cases with large tumors.Изучены клинические проявления и частота рецидивов у 90 больных с полными ремиссиями лимфомы Ходжкина в I – II стадиях в возрасте от 18 до 79 лет. Диагноз во всех случаях был подтверждён при морфологическом исследовании. Установлено, что лимфому Ходжкина чаще диагностировали в возрасте от 18 до 39 лет (76,7%). Первичный опухолевый очаг чаще располагался в средостенных (42,3%) и надключичных (33,3%) лимфатических узлах. Частота рецидивов составила 24,4%. Рецидивы чаще наблюдали при локализации первичной опухоли в средостенных (26,3%) и надключичных (26,7%) лимфатических узлах. Частота рецидивов коррелировала с раз-мерами первичной опухоли. Diagnosis and clinical particularities of indolent non-Hodgkin,s lymphomas.Au fost studiate aspectele clinice și de diagnostic la 68 pacienți cu Limfoame non-Hodgkin (LNH) indolente cu vârsta cuprinsă între 27 și 79 de ani (bărbați-38, femei-30). Diagnosticul a fost confirmat histologic și imunohistochimic. LNH limfocitare din limfocite mici au fost identificate la 28 pacienți, din celulele zonei marginale-la 24, foliculare-la 16. LNH limfocitare din limfocite mici și din celulele zonei marginale s-au dezvoltat mai frecvent la persoanele cu vârsta 50-69 de ani (75,0% și 62,5%, respectiv), iar LNH foliculare –la vârsta de 40-59 de ani (68,8%). Independent de tipul LNH focarul primar tumoral a fost localizat mai frecvent în ganglionii limfatici (48,5%), preponderent în ganglionii limfatici periferici (78,8%), după care a urmat în splină (35,3%). Metastaze extranodale au fost înregistrate frecvent în măduva oaselor. În LNH limfocitare din limfocite mici și în LNH din celulele zonei marginale afectarea măduvei oaselor a avut loc în 95,5% și 82,4% respectiv, iar în LNH foliculare-în 72,4% cazuri. Alte organe și țesuturi au fost interesate mult mai rar. Supraviețuirea de peste 5 ani a pacienților cu LNH limfocitare din limfocite mici a constituit 65,1%, în LNH foliculare -57,2% și în LNH din celulele zonei marginale- 44,3%. Management of malignant hematological diseases in the conditions of the COVID-19 pandemic

Introducere. Tratamentul hemopatiilor maligne include:
chimioterapie, radioterapie, imunoterapie, care induc o
imunosupresie şi, respectiv, prezintă un risc major de dezvoltare
al infecţiilor. Factori potenţiali de dezvoltare a infecţiilor
grave sunt: vârsta înaintată, imunodeficienţa secundară, leucopenia, neutropenia. Infecţia COVID-19 are o evoluţie
mult mai gravă la pacienţii imunocompromişi cu hemopatii
maligne. Forma gravă se dezvoltă de 2-3 ori mai frecvent,
decât la persoanele sănătoase. Mortalitatea pacienţilor cu
hemopatii maligne în asocierea infecţiei SARS-CoV-2 este
mai mare de 20%. Pacienţii cu hemopatii maligne au un potenţial
înalt pentru vindecare la efectuarea unui tratament
antitumoral intensiv, cu evitarea ulterioară a complicaţiilor
infecţioase, spre deosebire de pacienţii fără o terapie în deplin
volum. Totodată tratamentul hemopatiilor maligne este
necesar de efectuat la timp şi în volum deplin. Întârzierea
iniţierii chimioterapiei poate afecta negativ prognosticul pacienţilor
cu hemopatii maligne. Măsuri profilactice specifice
pentru infecţia SARS-CoV-2 la pacienţii cu hemopatii maligne
nu sunt. Este necesar de a efectua măsurile generale, care
includ măsurile sanitaro–igienice, izolarea la domiciliu, la
posibilitate activarea de la domiciliu şi testarea pentru infecţia
SARS-CoV-2. Obiectivul acestui articol este prezentarea
managementului hemopatiilor maligne în condiţiile pandemiei
COVID-19.
Material şi metode. Pentru a selecta date din literatură,
a fost folosită baza de date PubMed, utilizând cuvintele cheie:
,,COVID-19”, ,,hemopatii maligne”, ,,pandemie”, ,,management”.
Astfel, a fost sintetizată informaţia despre managementul hemopatiilor
maligne în condiţiile pandemiei COVID-19.
Rezultate. Tratamentul hemopatiilor maligne este necesar
de efectuat la timp şi în deplin volum, deoarece întârzierea
iniţierii terapiei poate afecta negativ prognosticul
pacienţilor cu hemopatii maligne. Tratamentul specific antitumoral
(chimioterapia, imunoterapia, radioterapia) este
însoţit de citopenii şi toxicităţi semnificative, care trebuie
să fie considerate ca factori de risc în infecţia SARS-CoV-2
şi, ca urmare, cu un efect negativ al evoluţiei bolii. Astfel,
administrarea chimioterapiei poate agrava hipersensibilitatea
pneumoniei indusă de COVID-19. În perioada pandemiei
este necesar de a selecta pentru utilizare acele scheme
de tratament, care ar limita vizitele pacienţilor la medic şi
acordarea priorităţii administrării preparatelor perorale sau
subcutane.
Concluzii. Managementul corect al hemopatiilor maligne
în condiţiile pandemiei COVID-19 va reduce numărul cazurilor
de infectare şi evoluţia procesului infecţios. Este obligatorie
gestionarea justă al posibilelor complicaţii infecţioase,
care se pot dezvolta în perioda sau după administrarea tratamentului
specific.Abstract
Introduction. The treatment of hematological malignancies
includes chemotherapy, radiotherapy, immunotherapy
that induce immunosuppression and, respectively, present a
major risk of developing infections. Potential factors for the
development of severe infections are: advanced age, secondary immunodeficiency, leukopenia with neutropenia. SARS-CoV-2
infection has a much more serious course in immunocompromised
patients with malignant hematological diseases. The severe
form develops 2-3 times more frequently than in healthy
people. Mortality in patients with hematological malignancies
associated with SARS-CoV-2 infection is greater than 20%. Patients
with malignant hematological diseases have a high potential
for healing when performing intensive antitumor treatment
with subsequent avoidance of infectious complications
as opposed to patients without full-volume therapy. At the
same time, the treatment of malignant hematological diseases
needs to be performed on time and in full volume. Delayed
initiation of chemotherapy may adversely affect the prognosis
of patients with malignant hematological diseases. There are
no specific prophylactic measures for SARS-CoV-2 infection in
patients with hematological malignancies. It is necessary to
carry out general measures, which include sanitary-hygienic
measures, isolation at home, possible activation at home and
testing for SARS-CoV-2 infection. The objective of this article
was to present the management of malignant hematological
diseases in the conditions of the COVID-19 pandemic.
Material and methods. The PubMed database was used
in order to select the data from the literature, using the key
words: “COVID-19”, “hematological malignancies”, “pandemic”,
“management”. Thus, the information containing the data on
the management of malignant hematological diseases in the
conditions of the COVID-19 pandemic.
Results. The treatment of hematological malignancies
needs to be performed on time and in full amount, because the
delay of initiating the therapy can negatively affect the prognosis
of patients with malignant hematological diseases. Specific
antitumor treatment (chemotherapy, immunotherapy, radiotherapy)
is accompanied by cytopenias and significant toxicities,
which must be considered as risk factors in SARS-CoV-2
infection and as a result of a negative effect on the course of
the disease. Thus, the treatment with chemotherapy may aggravate
the hypersensitivity induced by COVID-19 pneumonitis.
During the pandemic it is necessary to select for use those
treatment schemes, which would limit the visits of patients to
the doctor and to give priority to the oral or subcutaneous drug
administration.
Conclusions. Proper management of hematological malignancies
in pandemic with COVID-19 will reduce the number of
cases of infection and the evolution of the infectious process. At
the same time, it is mandatory to follow a correct management
of possible infectious complications, which may develop during
or after specific treatment
